and/or significant decline in function?

Call the primary physician or the social worker at the hospital to set up an evaluation for Medicare covered (rehabilitation) physical therapy.
ANSWER:
It appears that extensive strength and balance training is required by a licensed health care professional.
An evaluation by a private-pay physical therapy company is recommended to determine if such a service is required.

2 | Is there memory impairment, dementia or Alzheimer's Disease?

3 | Does the person need physical help getting out of a chair or moving to/from a bed or toilet?

4 | Is there a fear of falling?

5a | Does any of the following apply to the person?:

- History of falls
- Can't walk alone
- Still relying on a walker after more than 6 months

5b | Does any of the following apply to the person?:
|
- Diagnosed with Parkinson's Disease or similar
- Stroke survivor
- Congestive heart failure (or history of)
- Pacemaker
- Other significant health issue
ANSWER:
It cannot be determined exactly what is required but here are three possibilities:

- Simple strength and balance exercises by personal trainer
- Simple balance exercises through Tai-Chi
Getting an evaluation by physical therapist is advised to see if such a service is required.

6 | Is the person feeling weak?

ANSWER:
It appears general strength exercises is required to maintain current function. If the person is currently on a strength and balance program, it is recommended that the person continue to sustain the current level.
Otherwise:
- Perform own exercises. See our fall prevention exercises for some ideas.
- Join fitness club
- Join group class at housing facility
- Simple balance exercises through Tai-Chi
ANSWER:
It appears one-on-one strength training is required.
